The best feedback questionnaire I have seen so far.

The following is one of the best feedback questionnaire I have seen so far. One thing I have never seen in a feedback questionnaire is the follow through on a time commitment made which would say, "This questionnaire will only take 5 minutes".

The following questionnaire gave a time expectation of 2 minutes and the rating scale was 1 to 10 and I enjoyed doing this feedback questionnaire every bit of it for 2 minutes. The questions were focussed on the experience that the user had while interacting with the customer.

  2. Ensuring your needs are met
  3. Showing genuine interest in helping you
  4. Listening and correctly understanding your needs
  5. Communicating clearly and effectively
  6. Providing you with the guidance you need
  7. Demonstrating knowledge of your requirements
  8. Providing you with accurate and useful information
  9. Assisting you quickly and efficiently
  10. Sharing your sense of urgency
