The Best Feedback Questionnaire I Have Seen So Far

The following is one of the best feedback questionnaires I have seen so far. One thing that almost never happens in feedback questionnaires is the follow-through on the time commitment โ€” you know the kind: "This questionnaire will only take 5 minutes" โ€” and then 20 minutes later you are still going.

This one gave a time expectation of 2 minutes, used a simple 1 to 10 rating scale, and I genuinely enjoyed every bit of it. The questions were tightly focused on the actual experience of interacting with the person helping you โ€” not the product, not the company, not a net promoter score. Just the human interaction.

The Questions

  1. Being courteous, professional and polite
  2. Ensuring your needs are met
  3. Showing genuine interest in helping you
  4. Listening and correctly understanding your needs
  5. Communicating clearly and effectively
  6. Providing you with the guidance you need
  7. Demonstrating knowledge of your requirements
  8. Providing you with accurate and useful information
  9. Assisting you quickly and efficiently
  10. Sharing your sense of urgency

Ten questions. Each one measurable, human, and directly tied to the experience. No vague brand questions. No leading statements. No padding. And it genuinely took 2 minutes.

The best questionnaires are the ones that respect your time and ask only what they can act on. This one did both.
